Description

Nimesulide Pharmacodynamics
Nimesulide is a drug from the group of non-steroidal anti-inflammatory drugs. It has anti-inflammatory, analgesic and antipyretic effects. Like most NSAIDs, nimesulide shows its pharmacological action by inhibiting the synthesis of prostaglandins. In contrast to classical NSAIDs, it acts in the area of inflammation and inhibits mainly cyclooxygenase type 2 (COX 2) and, to a weaker extent, COX 1, inhibiting the formation of pathogenic pro-inflammatory prostaglandins. Nimesulide blocks the release of histamine from mast cells; inhibits the synthesis of elastase, metalloprotease and collagenase, protecting cartilage tissue from damage. By suppressing the release of tumor necrosis factor alpha (TNF-a), nimesulide reduces the effect of kinins.

Indications
For symptomatic therapy, reduction of intensity of pain and inflammation at the time of use (has no effect on disease progression):
rheumatoid arthritis;
Joint syndrome with exacerbation of gout;
psoriatic arthritis;
ankylosing spondylitis;
Osteochondrosis with radicular syndrome;
osteoarthritis;
Myalgia of rheumatic and non-rheumatic genesis;
Inflammation of ligaments, tendons, bursitis (including gusttraumatic inflammation of soft tissues);
Pain syndrome of different genesis (including in the postoperative period, at trauma,
algodysmenorrhea, toothache, headache, arthralgia, lumboishyalgia).

Contraindications .
Hypersensitivity, complete or incomplete combination of bronchial asthma, recurrent polyposis of nasal mucosa or paranasal sinuses.
Intolerance to acetylsalicylic acid and other NSAIDs (including history). Erosive and ulcerative lesions of the mucous membrane of the stomach and duodenum, active gastrointestinal bleeding, cerebrovascular or other bleeding, inflammatory bowel disease (Crohn’s disease, ulcerative colitis) in the acute phase.
Hemophilia and other blood clotting disorders, decompensated chronic heart failure (CHF).
Liver failure or any active liver disease, hepatotoxic reactions when using nimesulide in the anamnesis, alcoholism, drug addiction, concomitant use of other hepatotoxic drugs.
Severe chronic renal failure (CKD) (creatinine clearance less than 30 ml/min), advanced renal disease, confirmed hyperkalemia.
Period after aortocoronary bypass surgery, pregnancy, period of breast absorption, children under 12 years of age.
Lactose intolerance, lactase deficiency or glucose-galactose malabsorption.

Dosage and administration administration

  • Adults and children over 12 years of age orally, with plenty of water, preferably after meals. Minimum effective dose should be used for as short a course as possible. Adults orally, 1 tablet 2 times a day. In case of gastrointestinal tract diseases, it is desirable to take the drug at the end of a meal or after a meal. The maximum daily dose for adults is 200 mg. Patients with chronic renal insufficiency need to reduce the daily dose to 100 mg.
