Loperamide, by binding to opioid receptors of the intestinal wall (modulation of choline and adrenergic neurons via guanine nucleotides), reduces intestinal smooth muscle tone and motility (by
Inhibition of acetylcholine and Pg release). Slows down peristalsis and increases the time of passage of intestinal contents. Increases the tone of the anal sphincter, helps to retain feces and reduce urges to defecation.
The action of the drug falls quickly and lasts for 4-6 hours.
Indications
Symptomatic treatment of acute and chronic diarrhea of various genesis (allergic, emotional, medicinal, radiation; when changing diet and quality composition of food, in violation of metabolism and absorption;
as adjuvant in diarrhea of infectious genesis).
Stool regulation in patients with ileostoma.
Contraindications
Hypersensitivity to the drug, diverticulosis, intestinal obstruction, acute ulcerative colitis, diarrhea with acute pseudomembranous enterocolitis, as monotherapy – dysentery and other gastrointestinal infections; pregnancy (1st trimester), period of pregnancy.
(I trimester), lactation, children (under 6 years).
With caution: liver failure.

Application during pregnancy
There is no evidence that loperamide has teratogenic or embryotoxic effects.
During the first trimester of pregnancy the drug is contraindicated.
During the II-III trimesters of pregnancy the drug administration is possible if the estimated benefit of therapy for the mother exceeds the potential risk for the fetus. Administration during breast-feeding A small amount of loperamide may penetrate into the breast milk; therefore, it is not recommended to use the drug during breast-feeding.
Dosage and administration method
- Orally, chewed. In adults with acute and chronic diarrhea, the first dose is 2 tablets (4 mg) and 1 tablet (2 mg) after each bowel movement (in case of liquid stool); the maximum daily dose is 8 tablets (16 mg).
- In children over 6 years of age, in acute diarrhoea, the first dose is 1 tablet (2 mg) followed by 1 tablet after each bowel movement (in case of loose stools); the maximum daily dose is 3 tablets (6 mg).
- If stools are normalized or stools are absent for more than 12 hours, the drug should be discontinued.
