Aminophylline (Euphylline) tablets Pharmacodynamics
Bronchodilator, xanthine derivative; inhibits phosphodiesterase, increases accumulation of adenosine monophosphate (cAMP) in tissues, blocks adenosine (purine) receptors; reduces influx of calcium ions through cell membrane channels, reduces contractile activity of smooth muscles.
Relaxes bronchial musculature, stimulates respiratory center, increases its sensitivity to carbon dioxide and improves alveolar ventilation, which ultimately leads to reduced severity and frequency of apnea episodes. By normalizing respiratory function, it helps to saturate the blood with oxygen and reduce the concentration of carbon dioxide.
Aminophylline (Euphylline) tablets has a stimulating effect on heart activity, increases the force and number of heart contractions, increases coronary blood flow and increases myocardial oxygen demand. Reduces the tone of blood vessels (mainly vessels of the brain, skin and kidneys). It has a peripheral venodilator effect, reduces pulmonary vascular resistance, lowers the pressure in the “small” circulatory circle. Increases renal blood flow, has a moderate diuretic effect. Dilates extrahepatic biliary tracts.
Stabilizes membranes of mast cells, inhibits release of mediators of allergic reactions.
Inhibits platelet aggregation (inhibits platelet activation factor and prostaglandin E2 (PgE2), increases resistance of red blood cells to deformation (improves rheological properties of blood), reduces thrombosis and normalizes microcirculation.
Aminophylline (Euphylline) tablets has a tocolytic effect and increases the acidity of gastric juice. When used in high doses it has epileptogenic effect.

 

Indications
Bronchoobstructive syndrome of any genesis, bronchial asthma (the drug of choice for patients with asthma of physical effort and as an additional means for other forms), chronic obstructive pulmonary disease (chronic obstructive bronchitis, emphysema), hypertension in the small circle of circulation, “pulmonary” heart, apnea at night.

 

Contraindications
Hypersensitivity to the drug (including other xanthine derivatives: caffeine, pentoxifylline, theobromine), epilepsy, peptic ulcer and duodenal ulcer (acute stage), gastritis with hyperacidity, severe arterial hyper- and hypotension, tachyarrhythmias, hemorrhagic stroke, retinal hemorrhage, children (< 3 years).

 

Dosage and administration method
  • Dosage should be adjusted according to individual characteristics of each patient, taking into account clinical response and stable concentration of eufillin in blood serum.
  • Orally, adults should be prescribed 150 mg 3-4 times a day after meals.
  • Children over 3 years of age should be administered orally at a rate of 7-10 mg/kg per day in 4 doses.
  • The highest dose of eufillin for adults is a single dose of 500 mg; the daily dose is 1500 mg.
  • Higher doses for children over 3 years of age: single dose 7 mg/kg, daily dose 15 mg/kg.
  • Duration of treatment course is from several days to several months, depending on the course of the disease and tolerability of the drug.
  • To reduce toxic effects, the drug should be prescribed in the lowest effective doses.
