Nitrofural (Furacilin) Pharmacodynamics
Antimicrobial agent. Active against Gram-positive and Gram-negative bacteria (including Staphylococcus spp., Streptococcus spp., Escherichia coli, Clostridium perfringens). Effective with resistance of microorganisms to other antimicrobial agents (not from nitrofuran group).
It has a mechanism of action that differs from other chemotherapeutic agents: microbial flavoproteins restore the 5-nitrogroup; the resulting highly reactive amino derivatives change the conformation of proteins, including ribosomal, and other macromolecules, causing cell death. Resistance develops slowly and does not reach a high degree.
Pharmacokinetics
When administered topically and externally, absorption is insignificant. Nitrofural (Furacilin) penetrates through histohematic barriers and is evenly distributed in fluids and tissues. The main route of metabolism is reduction of the nitro group. It is excreted by kidneys and partially with bile.
Indications
External use: purulent wounds, bedsores, II – III degree burns, minor skin injuries (including abrasions, scratches, cracks, cuts).
Topically: blepharitis, conjunctivitis, boils of the external auditory canal, osteomyelitis, empyema of the paranasal sinuses, pleura (cavity lavage), acute external and media otitis, sore throat, stomatitis, gingivitis.
Contraindications .
Hypersensitivity, bleeding, allergic dermatosis. Pregnancy and lactation is allowed for use during pregnancy and lactation.
Dosage and administration
- Topically, externally.
- Topically as 0.02% (1:5000) aqueous or 0.066% (1:1500) alcohol solutions – irrigate wounds and apply moist dressings.
- Intracavitary (aqueous solution): empyema of accessory sinuses (including maxillary sinusitis) – washing the cavity; osteomyelitis after surgery – washing the cavity followed by wet dressing; pleural empyema – after removal of pus the pleural cavity is washed and 20-100 ml of aqueous solution is injected.
- To wash the urinary tract and bladder, an aqueous solution is used with an exposure of 20 minutes.
- In otitis – an alcohol solution heated to body temperature, 5-6 drops daily in the external auditory canal.
- Blepharitis, conjunctivitis – instillation of an aqueous solution into the conjunctival sac. For mouth and throat rinses 20 mg (1 tablet) dissolved in 100 ml of water.
- For preparation of aqueous solution 1 part of nitrofural is dissolved in 5000 parts of 0.9% sodium chloride solution or distilled water. Alcohol solution is prepared in 70% ethanol.
