Terizidone Pharmacodynamics
Terizidone is a bacteriostatic antibiotic with a broad spectrum of action, derived from D-cycloserine. The mechanism of action is based on disruption of the synthesis of the cell wall of microorganisms. Terizidone is active against Mycobacterium tuberculosis, Mycobacterium bovis, Mycobacterium avium, as well as Staphylococcus aureus and Staphylococcus epidermidis. Enterococcus faecalis, Escherichia coli, Citrobacter spp., Enterobacter spp., Morganella morganii, Klebsiella pneumoniae, Pseudomonas aeruginosa are also sensitive.
Minimum effective concentration of terizidone for Mycobacterium tuberculosis is 10-40 mg/l, for Staphylococcus spp – 8-32 mg/l, for Gram-negative bacteria of clinical significance – 50-250 mg/l.
Cases of the development of secondary resistance are rare. There is no cross-resistance between terizidone and other antituberculosis drugs.
Indications
Tuberculosis (various forms and localizations) in the treatment of drug-resistant forms of the disease.
Contraindications
– Hypersensitivity to terizidone, other components of the drug, to cycloserine;
– Organic diseases of the central nervous system;
– Atherosclerosis of cerebral vessels;
– Epilepsy and epileptic seizures (including anamnesis);
– Psychiatric disorders;
– Severe chronic renal failure (creatinine concentration greater than 2 mg/dL);
– Alcoholism;
– Pregnancy, breastfeeding;
– Childhood under 14 years of age;
– Lactose intolerance, lactase deficiency and glucose-galactose malabsorption.
With caution.
Elderly age, chronic heart failure, chronic renal failure.
Application during pregnancy and lactation
Pregnancy
The use of Terizidone during pregnancy is contraindicated.
Breast-feeding period
Terizidone enters the breast milk. If it is necessary to use Terizidone during lactation, breastfeeding must be stopped.
Dosage and administration
- It is taken orally. The capsules should be swallowed in whole with a small amount of fluid, at regular intervals during the day.
- Daily dose for adults and adolescents over 14 years old is 750-1000 mg (250 mg (1 capsule) 3-4 times a day, every 6-8 hours or as prescribed by a physician). For patients with body weight less than 60 kgs it is recommended to use 250 mg (1 capsule) 3 times a day, for patients with body weight over 60 kgs 250 mg (1 capsule) 4 times a day. The highest single dose of Terizidone is 250 mg. The maximum daily dose is 1000 mg.
- If creatinine clearance is less than 30 ml/min, it is recommended to take 1 capsule (250 mg) daily or 500 mg (2 capsules) 3 times a week. Since terizidone is excreted by hemodialysis, it should be used at the end of hemodialysis procedure. Terizidone is contraindicated if creatinine concentration is more than 2 mg/dL (see section “Contraindications”),
- The duration of therapy is 3-4 months.
