Antifungal agent for external use

Terbinafine (Fungoterbine) is used for the treatment of fungal skin infections, including athlete’s foot, inguinal epidermophytosis, fungal lesions of smooth skin (tinea corporis), caused by dermatophytes such as Trichophyton (including T. rubrum, T. mentagrophytes, T. verrucosum, T. violaceum), Microsporum canis and Epidermophyton floccosum.

Yeast infections of the skin, mainly those caused by the genus Candida (e.g., Candida albicans), particularly diaper rash.
Pityriasis versicolor, caused by Pityrosporum orbiculare (also known as Malassezia furfur).

Description

Terbinafine (Fungoterbine) Pharmacodynamics
Antifungal drug for external use, which has a broad spectrum of antifungal activity. In low concentrations, terbinafine has fungicidal effect against dermatophytes (Trichophyton rubrum, T.mentagrophytes, T.verrucosum, T.violaceum, T.tonsurans, Microsporum canis, Epidermophyton floccosum), yeasts (mainly C.albicans) and certain dimorphic fungi (Pityrosporum or Mallassezia furfur). Activity against yeast fungi, depending on their species, may be fungicidal or fungistatic.
Terbinafine specifically alters the early stage of sterol biosynthesis occurring in fungi. This leads to ergosterol deficiency and intracellular accumulation of squalene, which causes cell death of the fungus. Terbinafine acts by inhibiting the squaleneepoxidase enzyme located on the cell membrane of the fungus.
Terbinafine has no effect on cytochrome P450 system in humans and, respectively, on metabolism of hormones or other drugs.
Fungoterbin also has anti-inflammatory, antipruritic, keratolytic effects. Due to the presence of urea in the cream, the drug has a moisturizing effect and promotes deeper penetration of terbinafine into the skin.

 

Indications
Treatment of fungal skin infections, including athlete’s foot (“fungus” foot), inguinal epidermophytosis (tinea cruris), fungal lesions of smooth skin (tinea corporis), caused by dermatophytes such as Trichophyton (including T. rubrum, T. mentagrophytes, T. verrucosum, T. violaceum), Microsporum canis and Epidermophyton floccosum.
Yeast infections of the skin, mainly those caused by the genus Candida (e.g., Candida albicans), particularly diaper rash.
Pityriasis versicolor, caused by Pityrosporum orbiculare (also known as Malassezia furfur).

 

Contraindications
– Hypersensitivity to terbinafine or to any component of Terbinafine (Fungoterbine).
– Breast-feeding period.
– Children under 12 years of age in connection with insufficient data on efficacy and safety.
Caution
If any of the following diseases/conditions/risk factors are present, always consult a physician before using Terbinafine (Fungoterbine):
– hepatic and/or renal impairment;
– alcoholism;
– Inhibition of medullary hematopoiesis;
– tumors;
– metabolic diseases;
– occlusive diseases of the vessels of the extremities;
– pregnancy.

 

Dosage and administration

Externally.

Adults and children from 12 years:

Before applying the cream, the affected areas should be cleaned and dried. The cream is applied once or twice a day, a thin layer on the affected skin and adjacent areas and lightly rubbing. In infections accompanied by diaper rash (under the breasts, in between the fingers, between the buttocks, in the groin area), the place of application of the cream can be covered with gauze, especially at night.

The average duration of treatment:

– Dermatomycosis of torso, shins: 1 week, once a day.

– Dermatomycosis of the feet: 1 week, once a day.

– Skin candidiasis: 1-2 weeks, 1 or 2 times a day.

– Lichen multicolor: 2 weeks, 1 or 2 times a day.

Reduction of clinical manifestations is usually noted in the first days of treatment. If treatment is irregular or if it is discontinued prematurely, there is a risk of re-infection. If no signs of improvement are seen after one to two weeks of treatment, the diagnosis must be verified.

Regimen of dosing of Fungoterbin cream in elderly people does not differ from the above mentioned.

Administration in children.

It is not recommended to use Terbinafine (Fungoterbine) in children under 12 years of age.
