Diartrine Pharmacodynamics
Diacerein is an anthraquinoline derivative, a diacetylated derivative of reine.
Metabolized to the active metabolite reine, it inhibits the activity of interleukin-1, which plays an important role in the development of inflammation and cartilage degradation in osteoarthritis.
Diacerein inhibits the action of other cytokines, including interleukin-6, tumor necrosis factor? The action develops in 2-4 weeks.
Diacerein has analgesic and anti-inflammatory activity when taken orally.
Indications
Symptomatic treatment of osteoarthritis of the hip and knee joints.
Contraindications
Hypersensitivity to diacerein, other drug components or anthraquinone derivatives; inflammatory bowel diseases (ulcerative colitis, Crohn’s disease); intestinal obstruction; abdominal pain of unclear genesis; patients with liver diseases (incl. lactase deficiency, lactose intolerance, glucose-galactose malabsorption; pregnancy and lactation; children under 18 years of age.
Caution
Moderate renal insufficiency (CKR 30-60 ml/min).
Irritable bowel syndrome.
Patients aged 65 years and older.
Use during pregnancy and during breastfeeding
No studies confirming the safety of diacerein use during pregnancy and breastfeeding have been conducted. There are no data on the effect of diacerein on fetal development. Use during pregnancy and breast-feeding is contraindicated.
Dosage and administration
- Inside, with a meal, swallowed whole with water, 50 mg (1 capsule), 2 times a day (morning and evening).
- The treatment is carried out continuously for a long time or courses of not less than 4 months.
- Since diacerein may cause acceleration of intestinal transit during the first two weeks, it is recommended to start treatment with 1 capsule per day (in the evening, with meals) for four weeks. Then, after consultation with a physician, the dose may be increased to 100 mg (2 capsules) per day.
- In patients with moderate renal insufficiency (CKD 30-60 ml/min), the drug dose should be reduced by half to 50 mg (1 capsule) per day.
