Chymopsin Pharmacodynamics
Chymotrypsin, trypsin (Chymopsin) for external use is an enzyme preparation of proteolytic action, splits necrotic tissue without affecting the living tissue, due to the presence of specific anti-enzymes. Liquefies viscous secretions and exudates.

 

Indications
Bronchopulmonary diseases: bronchiectatic disease, lung abscess, atelectasis, exudative pleurisy (for liquefaction of viscous thick sputum and airway sanitation).
Diseases of the ENT-organ: purulent sinusitis, purulent otitis (acute, subacute, chronic), eustachianitis with viscous exudate, after tracheostomy to facilitate removal of thick viscous exudate, chronic rhinitis.
Burns, bedsores, trophic ulcers, purulent wounds (cleaning of purulent-necrotic masses, acceleration of healing).
In ophthalmology: corneal diseases (ulcer, herpetic keratitis); burns of the mucous membrane of the eye, obstruction of lacrimal ducts, sluggish healing of eyelid skin wounds.

 

Contraindications
Hypersensitivity to Chymotrypsin, trypsin (Chymopsin) for external use.
II-III degree chronic heart failure, malignant tumors, pulmonary emphysema with respiratory insufficiency, pulmonary tuberculosis (open form), liver dystrophy and cirrhosis, viral hepatitis, pancreatitis, hemorrhagic diathesis.
Do not inject into bleeding cavities, apply to ulcerated surfaces of malignant tumors.
With caution:
Empyema of the pleura of tuberculous etiology (resorption of the exudate may contribute to the development of bronchopleural fistula).
Pregnancy and lactation:
There are no data on the use of the drug during pregnancy and lactation.

 

Directions for use and dosages.
  • In case of respiratory diseases, the drug in a dose of 25-30 mg is dissolved in 5 ml of distilled water and introduced into the airways through the inhalation device, bronchoscope, endotracheal tube, tracheostomy tube. Number of inhalations – from 1 to 3 times a day for several days depending on indications. Antibiotics, bronchodilators may be added to the drug solutions to enhance the action of chemopsin.
  • In chronic rhinitis the nasal cavity is injected or irrigated with Chymopsin solution 2-3 times a day (5 mg in 5 ml of 0.9% sodium chloride solution). In chronic purulent otitis media complicated by cholestiotomy, after irrigation with 0.9% sodium chloride solution, Chymopsin 0.5% solution (in 0.9% sodium chloride solution) is put into the ear 2-3 times a day.
  • In case of third degree thermal burns, a thin layer of Chemopsin is put on the scab at the rate of 1 g per 100 cm of the wound surface and covered with a dressing moistened with 0.9% sodium chloride solution or 0.25% procaine solution. A moisture-proof dressing is put on top to slow down the drying of the dressings. The dressings are changed after 24 hours.
  • In treatment of purulent wounds and bedsores the drug is used in doses of 25-50 mg (in 10-50 ml of 0.25% procaine solution). Sterile gauze swabs are moistened with the solution and applied to the wound surface for 2 hours to 24 hours depending on the thickness of purulent necrotic masses. Dressings with Chymopsin are changed every 3-5 days.
  • In treatment of corneal ulcers, burns and keratitis Chymopsin is used in the form of eye baths with 1:500 solution daily for 2-3 days or as 0.25 % solution drops (prepared ex tempore) 4 times a day for 1-2 days with 2 drops.
  • For treatment of lacrimal duct obstruction and eyelid skin wounds the drug is used as 1% solution (prepared ex tempore) by irrigation of the wound and lavage of lacrimal ducts.
