Description

Bicillin-3 Pharmacodynamics
Combined bactericidal drug of 3 salts of benzylpenicillin: dibenzyl-ethylenediamine, procaine, sodium or potassium, having a long action. Suppresses the synthesis of the cell wall of microorganisms.
Active against Gram-positive microorganisms: Staphylococcus spp. (non-penicillinase forming), Streptococcus spp. including Streptococcus pneumoniae, Corynebacterium diphtheriae;
anaerobic spore-forming bacilli: Bacillus anthracis, Clostridium spp., Actinomyces israelii;
Gram-negative microorganisms: Neisseria gonorrhoeae, Neisseria meningitidis, Treponema spp.
Staphylococcus spp. strains producing penicillinase are resistant to the drug action.

Indications
Infections (in adults) caused by susceptible microorganisms: syphilis and other diseases caused by pale treponemes (yaws), streptococcal infections (excluding infections caused by group B streptococci) – acute tonsillitis, scarlet fever, wound infections, rust inflammation; rheumatism (prevention).

Contraindications .
Hypersensitivity to drugs of the group of penicillin and other beta-lactam antibiotics, to procaine.
With caution:
Pregnancy, lactation, renal failure, allergic diseases, including bronchial asthma, pollinosis (including history), pseudomembranous colitis.
Pregnancy and lactation:
Use during pregnancy is possible only when the estimated benefit to the mother exceeds the potential risk to the fetus.
If application of the drug during lactation it is necessary to decide on stopping of breast-feeding.

Directions for use and dosages

  • The drug is injected intramuscularly into the upper outer quadrant of the gluteal muscle in a dose of 300,000 units. If two injections are necessary they are made in different buttocks. Repeated injections are made 4 days after the previous injection.
  • The injections are given in a dose of 600,000 units once every 6 days.
  • In treatment of primary and secondary syphilis a single dose of the drug is 1.8 million units. A course of treatment consists of 7 injections. The first injection is given in a dose of 300,000 units; the second injection is given 1 day later in a full dose; further injections are given twice a week.
  • During treatment of secondary relapsed and latent early syphilis a dose of 300,000 IU is used for the first injection, and 1.8 million IU for subsequent injections. Injections are given twice a week. The course of treatment is 14 injections.
  • The suspension is prepared using sterile water for injection, isotonic sodium chloride solution or 0.25-0.5% novocaine solution (5-6 ml) prepared ex tempore. The solvent is injected into the vial slowly, at a rate of 5 ml in 20-25 s. The vial with the mixture is shaken gently in the direction of its longitudinal axis until a homogeneous suspension (or slurry) is formed. Bubbles on the surface of the suspension at the walls of the vial are allowed.
