Description

Pentamine Pharmacodynamics
Ganglioblocator, bisquaternary ammonium compound. It has hypotensive, arterio- and venodilator effects. It blocks n-cholinoreceptors of autonomic ganglia and therefore inhibits transmission of nerve excitation from preganglionic to postganglionic fibers of autonomic nerves. The autonomic ganglia also become insensitive to the excitatory action of a number of cholinergic stimuli (including acetylcholine, nicotine, lobeline, cytisin). By interrupting the conduction of nerve impulses through autonomic ganglia, ganglioblockers change the function of organs supplied with autonomic innervation. This results in BP lowering, which is mainly due to reduction of vasoconstrictor impulses to blood vessels and dilation of the peripheral vascular bed (primarily arterioles). Inhibition of impulse conduction by cholinergic nerve fibers leads to impaired accommodation, bronchial dilation, decreased motility of gastrointestinal organs, inhibition of gland secretion, increased heart rate, decreased tone of bladder. Inhibition of n-cholinoreceptors of adrenal chromaffin tissue leads to decreased release of adrenergic substances and weakening of reflex pressor reactions.
In high doses it can block n-cholinoreceptors of skeletal muscles and CNS.
Azamethonium bromide is somewhat inferior to benzogexonium in activity and duration of action.

 

Indications
Diseases and conditions caused by peripheral vascular spasms (including endarteritis, intermittent claudication).
As a part of the combined therapy in hypertensive crisis, pulmonary edema, cerebral edema.
In anesthesiology practice – for controlled arterial hypotension.
In urologic practice – during cystoscopy in men (to ease the passage of the cystoscope through the urethra).

 

Contraindications
Closed-angle glaucoma, acute phase of myocardial infarction, severe arterial hypotension, shock, liver and/or renal dysfunction, pheochromocytoma, degenerative diseases of the central nervous system, hypersensitivity to azamethonium bromide.

 

Dosage and administration

 

  • Used v / m or v / v. Dosage is determined individually depending on indications and specific clinical situation.
  • Maximal doses: single dose for intravenous injection – 150 mg, daily dose – 450 mg.
