Description

Trombital Forte Pharmacodynamics
Reduces aggregation, platelet adhesion and thrombosis by inhibiting the synthesis of thromboxane A2 in platelets. The antiplatelet effect persists for
7 days after a single dose (more pronounced in men than in women).
Acetylsalicylic acid reduces mortality and the risk of myocardial infarction in unstable angina, is effective in the primary prevention of cardiovascular disease, especially myocardial infarction in men over 40 years, and in the secondary prevention of myocardial infarction.
Inhibits prothrombin synthesis in the liver and increases prothrombin time. Increases the fibrinolytic activity of blood plasma and reduces the concentration of vitamin K-dependent clotting factors (II, VII, IX, X). Increases the risk of hemorrhagic complications during surgical interventions, increases the risk of bleeding during anticoagulant therapy.
Acetylsalicylic acid in high doses also has anti-inflammatory, analgesic and antipyretic effects.
At high doses, acetylsalicylic acid stimulates excretion of uric acid (disrupts its reabsorption in the renal tubules).
Blockade of cyclooxygenase-1 in the gastric mucosa leads to inhibition of gastroprotective prostaglandins, which may cause mucosal ulceration and subsequent bleeding.
Magnesium hydroxide contained in Trombital Forte protects the gastrointestinal mucosa from the effects of acetylsalicylic acid.

 

Indications
Primary prevention of cardiovascular diseases, such as thrombosis and acute heart failure in the presence of risk factors (e.g. diabetes mellitus, hyperlipidemia, arterial hypertension, obesity, smoking, old age).
Prevention of recurrent myocardial infarction and blood vessel thrombosis.
Prevention of thromboembolism after surgical interventions on the vessels (coronary artery bypass grafting, percutaneous transluminal coronary angioplasty).
Unstable angina pectoris.

 

Contraindications
Hypersensitivity to acetylsalicylic acid, excipients of the drug and other nonsteroidal anti-inflammatory drugs (NSAIDs); cerebral hemorrhage; susceptibility to bleeding (vitamin K deficiency, thrombocytopenia, hemorrhagic diathesis); gastrointestinal erosive ulcers (acute phase); gastrointestinal bleeding; bronchial asthma induced by taking salicylates and other NSAIDs; combination of bronchial asthma, recurrent nasal and paranasal sinus polyposis with intolerance to acetylsalicylic acid; concomitant use of methotrexate at a dose of 15 mg or more per week; pregnancy (I and III trimesters); lactation; glucose-6-phosphate dehydrogenase deficiency; severe renal insufficiency (creatinine clearance (CK) less than 30 ml/min); severe hepatic insufficiency (Child-Pugh class B and C); chronic heart failure class III and IV according to NYHA classification; children under 18 years old.
Caution. In case of gout, hyperuricemia, because low doses of acetylsalicylic acid decrease uric acid excretion. If there is a history of gastrointestinal ulcers or gastrointestinal bleeding. In case of hepatic insufficiency (grade A on the Child-Pugh scale). In renal insufficiency (CKG more than 30 ml/min). In bronchial asthma, chronic respiratory diseases, hay fever, nasal polyposis, allergic conditions, drug allergies. In patients with diabetes. In patients of advanced age. In the second trimester of pregnancy. In anticipated surgical interventions (including minor ones, such as tooth extraction), because acetylsalicylic acid may cause a tendency to develop bleeding within a few days after taking the drug. When concomitantly administered with the following medicinal products (see section “Interaction with other medicinal products”).

 

Dosage and administration

 

  • The drug is taken orally, with water. The tablet may be swallowed whole or chewed or crushed.
  • 1 tablet of Trombital Forte contains 150 mg of acetylsalicylic acid. If necessary, the daily dose of acetylsalicylic acid may be reduced to 75 mg, at the discretion of the physician (see section “Special directions”).
  • Primary prevention of cardiovascular diseases such as thrombosis and acute heart failure in the presence of risk factors (e.g., diabetes mellitus, hyperlipidemia, arterial hypertension, obesity, smoking, old age):
  • 1 tablet once daily. If necessary, in case of prescribing by physician the scheme of daily administration of acetylsalicylic acid in dose of 150 mg and then in dose of 75 mg once a day, during first 24 hours we should take 1 tablet of preparation Trombital forte with 150 mg of acetylsalicylic acid and then 1 tablet of Trombital with 75 mg of acetylsalicylic acid once a day (see chapter “Indications”).
  • Prevention of recurrent myocardial infarction and blood vessel thrombosis: 1 tablet once daily.
  • Prevention of thromboembolism after surgical interventions on the vessels (coronary artery bypass grafting, percutaneous transluminal coronary angioplasty): 1 tablet once daily.
  • Unstable angina pectoris: 1 tablet once daily.
  • The drug is intended for long-term use. The duration of treatment with Trombital Forte is determined by the physician.
  • Use the drug only in accordance with the indications, the method of administration and the doses stated in the instructions.
